SERVICES & FEES FOR OUR DIRECT PRIMARY CARE PRACTICE

Services covered by the Monthly Subscription

Medical Services (for an average patient this will typically cover 70% to 80% of all health treatment needs )

  • Family Medicine: treatment support for illness symptoms and ongoing care
  • care coordination,
  • patient advocacy,
  • Integrative Medicine: wellness education and preventative care
  • Acupuncture – (up to 4 visits per year included in the subscription)

Access Services

  • Access for unhurried Face-to Face Visits (up to 24 per year after which a $75 per visit charge applies)
  • Access for email and text consultations
  • Same or next day appointments and message follow-up is the norm

MONTHLY SUBSCRIPTION FEES

 

Registration Fee: is a $50 fee paid once at the beginning of the subscription for every patient Monthly Subscription Fees: The Monthly Fee depends on the age(s) of the member(s) in a family / household unit and are billed in arrears at the end of every month:

  • Age 0-5:        not eligible for membership
  • Age 6-25:      $20 per month
  • Age 26-35:    $35 per month
  • Age 36-50:    $55 per month
  • Age 51-62:    $80 per month
  • Age 63+:       not eligible for new membership (including Patients that are 65 and under that are covered by Medicare, Medicaid, or Tricare)
  • Household/Family discount: Once 4 members from the same household have become active subscribers a 20% discount will apply to all monthly rates.

Ancillary Services
  • Labs ordered for patients under Heritage Direct ‘s account (where Heritage Direct pays the lab on behalf of the patient)
  • Specific Medications and Supplements dispensed from Heritage Direct’s office (Coming Soon)
  • Radiologic testing where Heritage Direct is charged on behalf of the patient rather than the patient’s insurance
  • Pathology testing where Heritage Direct is charged rather than the patient
  • Medical supplies for various procedures

ANCILLARY SERVICES FEES

Ancillary Fees are paid for on an “as utilized” basis
  • Ancillary Fees are charged for services and products not covered by the Monthly Subscription Fees.
  • Rates for these services are posted in our office
  • Common labs are made available at wholesale rates plus a small handling charge. 
  • In office dispensing of medications provides not only a convenience for our patients but also helps them save money…. Coming Soon!

Services NOT provided by Heritage Direct Care, LLC:

Although Heritage Direct physician(s) will coordinate medical care in his or her sole discretion with specialists, the fees paid under this Agreement do not include or cover any specialists’ charges, charges by any provider other than those practicing at Heritage Direct, or any health care items (such as durable medical equipment). The following items are not provided by Heritage Direct Care:

  • Care from Specialist Physicians
  • Hospital and Emergency Room Care
  • Medications from Pharmacies /Hospitals*
  • Dentistry
  • Chiropractic care
  • Naturopathy
  • Hospice care
  • Nursing home or rehabilitation facility care
  • Home health care

* except for those medications dispensed from Heritage Direct’s office or via Heritage Direct’s mail services

  • Physical therapy
  • Massage therapy
  • Maternity/prenatal care and delivery
  • Vision and hearing care
  • Cosmetic surgery
  • Podiatry care
  • Outpatient diagnostic procedures such as: x-ray, CT scan, MRI, mammogram, ultrasound, colonoscopy, Endoscopy
